Reentry in a pulmonary vein as a possible mechanism of focal atrial fibrillation.

چکیده
The case of an 18-year-old woman with recurrent idiopathic catecholamine-sensitive paroxysmal atrial fibrillation is reported. Recordings of multiple initiations of atrial fibrillation at the proximal part of the right superior pulmonary vein suggested local reentry in the vein as the mechanism of atrial fibrillation. A single radiofrequency pulse delivered at this site resulted in definite cure of the arrhythmia.

Is there a connection?
Ablation of persistent atrial fibrillation (AF) remains challenging, particularly as the duration of continuous AF before the procedure increases. Attainment of durable pulmonary vein (PV) isolation remains a fundamental goal in this population; continuous retriggering from focal peri-PV sources (both automaticity and localized reentry) as a potential mechanism for maintaining persistent AF was...
متن کاملMechanism underlying initiation of paroxysmal atrial flutter/atrial fibrillation by ectopic foci: a simulation study.
BACKGROUND The mechanisms underlying paroxysmal atrial flutter/atrial fibrillation initiation by ectopic foci from various locations are unclear. METHODS AND RESULTS We used parallel computational techniques to study an anatomically accurate 3-dimensional atrial structure incorporating a detailed ionic-current model of an atrial myocyte. At the single-cell level, upregulation of the L-type Ca...

Atrial fibrillation (AF) is the most common cardiac arrhythmia in the United States, affecting an estimated 2.66 million people as of 2010. However, despite the high prevalence of this disease and the heavy socioeconomic burden associated with it, treatment options remain limited, especially in comparison with other reentrant arrhythmias. Of the available treatment options, catheter-based ablat...
متن کاملArrhythmogenic substrate of the pulmonary veins assessed by high-resolution optical mapping.
BACKGROUND It has recently been recognized that atrial fibrillation can originate from focal sources in the pulmonary veins (PVs). However, the mechanisms of focal atrial fibrillation have not been well characterized. We assessed the electrophysiological characteristics of the PVs using high-resolution optical mapping. METHODS AND RESULTS Coronary-perfused, isolated whole-atrial preparations ...
متن کاملArrhythmias in Patients With Pulmonary Hypertension; A Narrative Review of the Current Literature
Incidence and prognostic relevance of supraventricular arrhythmias were evaluated in nine studies. These studies investigated patients with different forms of pulmonary hypertension. Supraventricular arrhythmias were more often found in patients with group two pulmonary hypertension. Common findings in these patients were elevated right atrial pressure and diameters and reduced tricuspid annula...
